Employee's fingertips amputated in carding machine rollers

At approximately 5:30 p.m. on December 19, 1997, Employee #1, of Sentinel Consumer Products in Clearfield, UT, was operating a carding machine when some loose cotton became stuck between two drums/rollers. He removed the guard and was trying to remove the material when his right hand became caught between the drums/rollers, partially amputating the tips of his first three fingers.
