Event: 11/21/1997
Employee killed when struck by falling I-beam

Employee #1 was grinding the end of a pipe when a 10 ft long steel I-beam was swung over the area where he was working. The beam was being held with a 5/16 in. thick choker wire rope. Employee #1 saw the beam and started to leave his work area when the choker broke and the beam fell on his head. He suffered a fractured skull and was killed instantly.
