Investigation Summary
At approximately 12:00 noon on October 22, 1996, Employee #1 was tearing down a small concession stand. He was working on the roof and his wife was working on the ground. She later stated that Employee #1 appeared to lose consciousness, fold up, and then fall approximately 9 ft to the ground, striking his head on the concrete surface. He was killed.
