Employee injured when trench caves in

At approximately 2:45 p.m. on Tuesday, September 3, 1996, Employee #1 was in a 5 ft 8 in. deep by 7 ft 3 in. wide trench in the northwest parking lot of Utah Valley State College in Orem, UT. He was retrieving a target from the end of a 24 in. (inner diameter) concrete pipe when the side of the trench failed. Employee #1 was buried to above his knees. No form of cave-in protection was in use.
