Investigation Summary

Investigation Nr: 171054273
Event: 06/17/1996
Two employees burned during repair of industrial roller

Employees #1 and #2 were using an oxy-acetylene torch to repair a roller when they started a small fire on the roller motor housing. Employee #1 picked up a small fire extinguisher to put out the fire, but his coveralls were stained with grease and oil, and while trying to extinguish the blaze he caught fire himself. Employee #2 went to assist him, but he, too, was wearing coveralls stained with oil and grease, and he caught on fire. The fires on both employees were extinguished, and they were flown by helicopter to the University of Utah Burn Center.
