Investigation Summary
On September 25, 2017, an employee was crouching down in the floor of an excavation, installing a wood ledger with a handheld cordless drill, adjacent to an unstable concrete wall. Another employee was nearby in the excavation handing tools. The wall tipped over onto the employee, resulting in 7 broken ribs, 5 fractured spinal bones, displaced hip, cuts, scrapes, and bruises on face, chest, hands and legs.
