Investigation Summary
At approximately 8:00 a.m. on April 4, Employee #1 was using a Delta Shaper to make panel doors. He was routing a panel without the Seco pressure feed rollers in place when the panel slipped. The tips of the first three fingers on his left hand struck the shaper blade and were amputated to the first knuckle. Employee #1 was taken to Ogden Medical Center for treatment.
