Investigation Summary
At 5:30 p.m. on April 24, 1996, Employee #1 was working on a link arm bushing press, which was secured to the table by two C-clamps. As a part was being inserted into the press, and the link arm actuated the press' downward stroke, the press began to slide. Employee #1 reached behind the press to pull it toward her or to steady it. As she did so, her left ring finger passed between the top plate of the press and the plastic safety shield that was secured to the press plunger assembly, just as the plunger started its return from the downward stroke. Her left ring finger became caught and was crushed between the plunger and the top plate. Employee #1 was given first aid at the site and then transported to Logan Regional Hospital, where she was treated for a crushed and fractured finger.
