Investigation Summary
At 3:15 p.m. on November 14, 1995, Employee #1, of the Smithfield Division of Icon, was operating an articulating, powered industrial lift truck. He was driving in reverse, with his left foot hanging outside the lift. He was turning a corner when his foot caught the corner of a metal storage rack frame. Employee #1 suffered multiple fractures of the left metatarsals and was transported by ambulance to Logan Regional Hospital.
