Employee fractures foot when struck by falling steel beam

Employee #1 and a coworker were shaking out a pile of steel beams, using one shake out hook instead of two. Employee #1 was at the raised end. When he walked to the other end to help his coworker move the grounded end, the beam slipped out of the hook and fell on his foot. Employee #1 sustained fractures to his foot and was hospitalized.
