Employee injured in fall from roof

On December 1, 1995, Employee #1, of Cascade Gypsum & Building Supply, Inc., Bend, OR, was delivering roofing material to a construction site. After placing the pallet of material on the roof, he went up to the peak to help another worker. Employee #1 stepped on the unfinished side of the roof and began to slide. No toe boards or fall protection were in place. Employee #1 fell 18 ft 1 in. from the roof and was then thrown 15 ft 10 in. away from the house. Suffering multiple fractures, he required hospitalization.
