Employee's fingers injured in table saw

Employee #1 was setting up a table saw in the mill of a mobile home manufacturing plant in preparation for ripping several 10 to 12 ft long pieces of hardwood into 3/4 in. strips with a 22 1/2 degree angled edge saw. He checked the setting of the fence by running a test cut through the saw. He had already pushed the material part way through the blade and was at the back of the saw pulling the material the rest of the way through when the material kicked back. His hand was pulled into the blade, amputating the tip of one finger and lacerating another finger. The saw was not equipped with a hood guard, spreader, or anti-kick-back devices, and its fence was not securely attached. Subsequent investigation of the accident determined several contributing causes, including inadequate (1) saw guarding; (2) training of employees; (3) supervision of employees; (4) work place inspections; and (5) training of the safety committee.
