Investigation Summary
At approximately 8:50 a.m. on September 20, 1995, Employee #1 and his foreman were crossing the roof to get a load of tar when they saw a piece of plywood lying on the roof. Employee #1 asked his supervisor if he wanted him to get the plywood and lay it over the roof tie-in (the elevation difference between the part of the roof that had roofing installed and the part where the bare metal roof began). The foreman said, "It's a good idea." Employee #1 picked up the north side of the 4 ft. wide by 8 ft. long by 3/4 in. thick sheet of plywood and stepped forward into the 49 in. square roof opening that the plywood had been covering. Employee #1 fell approximately 35 ft to the concrete floor and was killed.
