Investigation Summary
At approximately 11:15 a.m. on October 30, 1995, Employee #1 and a coworker were on a job-made work platform installing soffit on a dormer over the garage of a two-story single family house under construction. The platform consisted of an aluminum ladder with a 2 by 8 across the rungs for a walk board supported by a length of 2 by 6 nailed on either side of the dormer. The platform was approximately 14 1/2 ft above the ground and was 14 ft 9 in. long by 16 1/2 in. wide. Employee #1, who weighed approximately 250 lb, was on the left end of the platform and the coworker was in the middle walking toward the right end. The left end of the platform gave way and Employee #1 fell to the gravel drive, incurring serious head injuries. The coworker was able to grab the bottom of the dormer window opening and pull himself into the house. Employee #1 was hospitalized and later died at approximately 8:30 a.m. on November 7, 1995. The platform had not been constructed to support at least four times the maximum intended load, as required.
