Event: 02/13/1996

One employee killed, two injured when struck by falling deck

Employee #1, a laminator, was the leader of a crew including Employees #2 and #3 and a coworker. They were turning a 40 ft long, 1,300 lb deck in a 400 mold when it came out of the U-shaped bracket and fell on Employees #1 through #3. Employee #1 suffered internal injuries and was killed. Employee #2 sustained a broken nose and contusions, and Employee #3 suffered contusions and a head laceration.
