Employees burned when sprayed by hot water-turpentine mix

At 7:40 a.m. on April 30, 1996, a subcontractor began to take out a flange from a digester prior to removing the digester from the line. The operating personnel had prepared the line to be shut down by locking it out. When the flange was removed, a mixture of hot water and turpentine sprayed out of the flange for approximately 5 minutes, soaking Employees #1 through #4. All four employees suffered second-degree burns, and Employees #1 and #2 were hospitalized. The accident resulted from failure to drain the line. A citation was issued for failure to follow the company's lockout/tagout policy and procedures.
