RACK OVERTURNED ON EMPLOYEE

Two employees were pulling a metal rack filled with yarn. The floor of the work area had small holes in the concrete floor and uneven metal grating over drainage ditches. The metal rack was unbalanced because the yarn was only on one side, with nothing on the other side. When the wheels hit the uneven floor, the rack overturned on one of the employees. The employee suffered a fractured pelvis. He died of his injuries.
