Employee's forearm crushed when caught in printing press

Employee #1, a worker at a printing company, was adjusting a two-color Heidelberg offset printing press, model SORZ. The press had run about 200 prints when it began to develop a scumming problem. With the press running, Employee #1 reached through a 2 in. by 10 in. opening for the interlocked barrier guard to adjust the dampening ink unit. He was withdrawing his hand when he slipped and fell. His hand and forearm were pulled back into the roller nip point and crushed. Employee #1 was extricated from the press and taken to the hospital, where his right hand was amputated just above the wrist.
