Employee killed when struck by joist

Employee #1 and coworkers were helping to place an approximately 1,000 lb joist into a paint dip tank. The joist was being hoisted by a crane using shop-made L-shaped hooks. As the joist was being pushed toward the dip tank, it slipped off the hooks and struck Employee #1 above his right eye, causing him to fall backward onto the nonpowered rollers. He died of massive head trauma. The shop-made hooks had not been load tested to determine the maximum weight they could safely hold. One was bent at slightly more than a 90 degree angle, which allowed the load to slip and fall.
