Employee killed when struck by falling box press

Employee #1 was pushing boxes down a conveyor, which required him to place his body under an air operated box press. The 600 lb press plate broke and fell onto Employee #1, crushing and killing him. The press plate was attached to the push shaft by threads, and the female part on the plate itself was made of cast; when it broke the plate fell.
