Employee's fingers amputated in mechanical power press

Employee #1 was operating a 30-ton, full revolution, pedal-operated mechanical power press. She was being monitored by a fitter and had machined about ten parts when her right hand became caught in the point of operation and four of her fingers were amputated. She was taken to the hospital, where one finger was reattached. The press was outfitted with button-type harness restraints, worn on the hands, that should have kept Employee #1's fingers 3 in. from the point of operation. There was no evidence that the right hand harness had malfunctioned, nor did it have any blood or other marks indicating that Employee #1 had been wearing it at the time of the accident. Apparently, the harness had either fallen off Employee #1's hand, or she had removed it.
