Investigation Summary
Employee #1, of All Roofs N.W., Inc., was removing a 5 gallon bucket of tar that was suspended inside a tar kettle. He slid a broken mop handle under the handle of the bucket to lift it out of the kettle. The bucket slid down the mop handle and the contents spilled onto the employee's hands and stomach and splashed up onto his face. He was transported to the Portland Burn Center, where he was admitted with second-, third-, and fourth-degree burns.
