Investigation Summary
Employee #1 was feeding uncooked dough into a slicer for cookies. The slicing function was being done by a band saw used to cut several stacks of dough at a time. The blade was not properly covered, shielded or guarded. Employee #1 was attempting to clean or remove excess dough from machine. The blade was still running, and Employee #1 cut several fingers and the edge of the palm of his hand.
