EMPLOYEE'S LEG IS CRUSHED BETWEEN FORK LIFT AND A CONTAINER

Employee #1 was in the process of hooking a chain to a trash container from a forklift when the driver engaged the vehicle without warning him. Employee #1 was unable to get out of the way and was caught between the container and the forklift. He was hospitalized for a fracture to his leg. The employer had not ensured proper communication between the driver and the other employees.
