Employee burned when lacquer thinner ignites

At approximately 6:15 a.m. on February 6, 1997, Employee #1, of Acme Vial & Glass Company Inc., was using lacquer thinner to clean an exit chute on a vial forming machine. He had the flammable liquid in a butter container near a flame on the machine when the vapors caught on fire. Employee #1 pulled back, spilling the liquid out of the container and onto himself. He caught on fire and sustained burns on his face, wrist, and shoulder. These areas were exposed and were not covered with material as was the rest of his body.
