Employee injured in fall from elevated work platform

At approximately 9:15 a.m. on June 1, 1996, Employee #1 was placing rods on the bottom side of trusses that were 21 ft 6 in. above the ground. He was working from a movable work platform that was elevated to 15 ft 10 in. He put the platform in reverse, then stopped and began walking to the rear of the lift. The shift of his weight on the platform caused the left rear wheel to drop into a 24 in. diameter by 2 in. deep hole. As the platform started to tip, Employee #1 tried to get on the guardrail to jump. His timing was off and he landed on the concrete floor, suffering multiple injuries.
