Investigation Summary

Investigation Nr: 202540423
Event: 04/15/2009
Employee Cuts Finger With Saw

At approximately 7:50 p.m., on April 16, 2009, Employee #1, one of the members of the flooring crew, was assigned to repair a floor that was damaged by water. The location was at the main corridor at the 5th floor leading to suite 500 at 200 Med Plaza Los Angeles CA 90095. Employee #1 started tearing out the damaged floor using a screw driver and a ply bar. The area involved in this type of activity was 3 feet by 4 1/2 feet. Employee #1 started prepping the floor and was removing the residual glue on the floor using a razor to smooth the surface. He then started dry fitting the area and applied the glue before starting to cut the wood into the desired dimension. At around 7:50 p.m., while Employee #1 was cutting wood using a powered chop saw, he observed that the saw blade began to bind with the wood that was being cut. He used his left hand to hold the wood material and at the same time lifted the chop saw by pulling it and while doing this, the chop saw kicked back and nipped the tip of his left index finger. Employee #1, together with a coworker immediately walked to the emergency room of the UCLA Ronald Reagan Medical Center which was located nearby. Employee #1 was seen and treated by the emergency physician and was released at 2:30 a.m. the following morning. He was given prescribed medication on this treatment and the confinement was less than 24 hours.
