Employee's Shoulder Is Fractured in Fall from Ladder

On March 19, 2009, Employee #1, a pipefitter, worked for an HVAC contractor that was contracted to install an HVAC system. He was using a 14 to 28 ft Louisville extension ladder, Part Number 911146D, to gain access to HVAC piping. He did not tie or secure the ladder believing that the job would be of short duration. He climbed facing the ladder and reached the 6 or 7th step. He leaned to his left to place his tools on a pipe to start working and the ladder slid to his right and fell. He fell approximately 6 ft and was hospitalized with a fractured shoulder.
