Investigation Summary

Investigation Nr: 201164589
Event: 03/14/2003
Employee struck by falling I Beam and injured

At approximately 2:00 p.m. on March 14, 2003, an employee of Ammco Ornamental Iron, Inc. was working in the raw material storage location. In this storage area, heavy long steel beams were aligned lengthwise in north-south direction, in rows without any space in between them. The steel beams were stock piled one on top of the other with 3.5-square-in. wooden blocks placed randomly between them at various intervals and height. The timber blocks used as spacer below 40-ft-long, 12-in-wide steel I beam, weighing approximately 2,455 pounds, collapsed. The beam fell from its storage location and hit the employee on his hip and pinned him down against the ground. The employee was hospitalized and treated for fractured left tibia.
