Investigation Summary

Investigation Nr: 202539722
Event: 06/16/2008
Employees Fall from Balcony, One Hospitalized with Fractures

At approximately 2:30 p.m. on June 16, 2008, Employee #1 and Employee #2 were on the second floor balcony and tried to pull an extension ladder up from the ground for their skylight installation. According to both employee statements, the permanent guardrails were installed earlier on that day and were not totally secured. Both Employee #1 and Employee #2 bent over the guardrails to pull the ladder up. While they were pulling the ladder, the guardrails gave way. They lost their balance and fell 12 ft from the balcony to the ground. There were no actual witnesses to the accident. Both employees were taken to the hospital. Employee#1 suffered minor back injury and returned to work after two days. However, Employee #2 suffered multiple and compound fractures to both his feet and ankles and was hospitalized for extensive physical therapy.
