Investigation Summary
On May 31, 2008, Employee #1 was working at a hot forging company. He was driving a key into the dies of a drop forge swinging a sledge hammer in a sideways fashion. As he hit the locking key, a piece of it flew off and struck him in the face. It then ricocheted off his cheek and struck the inside of his safety glasses. It then lodged in his left eye. Employee #1 was hospitalized.
