Investigation Summary
At approximately 1:20 p.m. on March 11, 2008, Employee #1, a tower crane foreman, was getting ready to rig a trash box with the Tower crane Number 1. Employee #1 was preparing to fly the trash box upside down, which is not the normal procedure for moving these trash boxes. Employee #1 instructed the forklift operator to hold the trash box 3-ft to 4-ft above the ground. Employee #1 then brought the crane down, and rigged one side. The forklift operator then yelled that the trash box was moving. Employee #1 spun to the left to get out of the way. The box caught Employee #1 on the right shoulder and pushed him to the left into a similar trash box. Employee #1 sustained a fracture to his left leg and left elbow. Employee #1 was hospitalized for three days.
