Investigation Summary
At approximately 9:00 a.m. on May 22, 2007, Employee #1 was working in the wood cutting department. He was using a Whirlwind Model 212 semi-automatic pneumatic cut-off saw manufactured by Sehng Feng Machine Co in Taiwan. He was cutting wood and his left index finger was stuck by wood, causing him to step on the foot pedal. The clamp came down and the blade came up, cutting his fingers. His left index, middle, and ring finger were amputated and he was hospitalized.
