Investigation Summary
On September 6, 2007, an employee ascended a wooden ladder that served as access to the roof of the Drake Pool Building. The ladder was positioned under a hatch on the second floor, mezzanine, of the building and had allegedly been in place for over 15-years. The employee opened the hatch in order to allow a film crew onto the roof. The film crew was comprised of various parents of the school's football team members and it was customary to allow various persons onto the roof of the Drake Pool building for better viewing of the athletic events occurring in the athletic field adjacent the building. The hatch cover was padlocked with a master key lock to which all teachers and staff had a key. Prior to the accident, the subject ladder and roof hatch access had reportedly been routinely used by various contractors, faculty, and staff members as well as parents and students to access the roof to not only view the athletic events, but to provide routine maintenance and repair to the solar panels and HVAC equipment that was permanently installed on the roof. The subject ladder was a portable wood ladder was 16-ft 2-inches long and was positioned at an angle. The rails of the ladder rested just below the hatch cover and the feet rested on a smooth concrete surface. The ladder was of the old-school agility ladder type that had previously been wall mounted in the gym and used for stretching. The ladder design was not intended to be used as a portable ladder as both ends of the rails were smooth and rounded. The employee at the top of the ladder and was opening the hatch cover when the ladder slipped causing him to fall to the concrete floor below. The employee suffered numerous fractured bones and various internal injuries. He was hospitalized in intensive care for two-weeks. He returned to work 4-months after the accident and continued ongoing treatment for his injury.
