Employee Is Injured in Fall Into Tank

On December 16, 2008, Employee #1 was removing old piping inside a tank so the tank could be demolished. The employee set up the fall protection tripod on the top of the tank. He lowered himself into the tank and asked a coworker, the attendant, if he was hooked up. He was not, and he fell approximately 15 feet to floor of the tank. He injured his ribs and was transported to Industrial Health Care Clinic in Selma, CA, for treatment but did not require hospitalization. Further investigation revealed that Employee #1 did not follow the company removal plan and enter the tank from the maintenance port hole located at the bottom of the tank, instead he lowered himself into the tank from the top.
