Investigation Summary
At approximately 3:30 p.m. on January 30, 2003, Employee #1, a foreman for ACX Pacific Inc., was working at the company hay bale compressor plant. He was on an elevated work platform trying to align hay bales as they were fed into the bale feeder. The bale feeder arm moved in reverse and the gear rolled over his left hand. Emergency medical personnel were called, and he was transported to Kern Medical Center, Bakersfield, and then to USC Medical Center, Los Angeles, where he was hospitalized and treated for multiple finger amputations to his left hand.
