Employee Fractures Foot When Caught in Machine

On October 1, 2007, Employee #1 was working on a carpet shredding machine. A large piece of carpet got stuck in the rollers on top. Employee #1 was told to climb on the machine and try to pull out the carpet. Employee #1 was not told to turn off the machine. When Employee #1 pulled the carpet out of the machine it started up and pulled his left leg in crushing his ankle and leg. Employee #1 sustained fractures and was hospitalized.
