Employee Fractures Hand When Caught in Bucket Elevator

On April 21, 2009, Employee #1 was cleaning a hopper/conveyor on the process line in a pistachio nut processing plant. He turned off the conveyor, but did not tag out the switch. While he was cleaning the hopper/conveyor, a coworker traveled by him. The coworker needed to run the line, but did not see Employee #1 and turned on the machine. Employee #1's right arm was caught in the conveyor and hopper, fracturing his hand. He received treatment at Sierra View Hospital in Porterville, CA and was hospitalized.
