Investigation Summary
On July 24, 2009, Employee # 1 was cleaning sorting room conveyors. She was not using extension tools nor did she turn off the conveyors. As she was wiping a conveyor belt near the tail roller, the roller caught her cleaning rag and pulled her into the rollers. Employee #1 was hospitalized with a fractured arm.
