Investigation Summary
At approximately 1:30 p.m. on November 23, 2002, Employee #1 and two coworkers were sent to the Test Dept to do a motor alignment on a pump. Employee #1 was at the back of the pump aligning the motor and the pump. When Employee #1 finished he step down from the pump base and lost his footing. Employee #1 fell and broke his wrist. Employee #1 was hospitalized.
