Employee injured when finger amputated in machine roller

On November 21, 2000, an accident occurred in the production area of the J. Miller Company. Employee #1, a machine operator, was cleaning the rolls of a "gluer" machine which is used to glue cardboard pieces together to be used as shipping material. The machine was not locked out or deenergized and was running while Employee #1 wiped the rolls with cleaning solution and paper towels. The paper towel got stuck to Employee #1's hand and then got stuck to the roller, pulling her hand into the roller, which crushed and severed her right index finger. Employee #1 was taken to St. Joseph Hospital for emergency treatment.
