Investigation Summary
At about 7:30 a.m. on March 3, 1998, Employee #1, who was a machine operator and worked at the Site Company, sustained a partial amputation of the right middle finger at the second digit. At the time of accident Employee #1 was running press Number 7, 50-Ton V and O Full-Revolution Press, Model Ps, Serial Number 50-5669. On the day of the accident, Employee #1 started his work shift at 6:30 a.m. He turned on the machine and let it warm up before starting to run parts. Then at about 7:00 a.m. he ran one part and inspected for quality assurance. He verified that the part met the expectations and then started production. He was running small pieces of steel that were designed for pedals of golf carts brakes. The parts were approximately 5 to 6 in. long by 2 in. wide. Employee #1 was running two parts at a time. He put two parts, activated the press with two hands, removed the finished parts, and then started the cycle again. On the third time after he started production, while he was retrieving the parts from the point of operation area, the press double tripped and smashed and partially amputated his right middle finger. Employee #1 was transported to the Technimed-Vernon Industrial Medical Center and then referred to the Western Hand Center, where he was treated for his injury. Employee #1 had nine months of experience as a press operator. The employer was involved in the stamping and manufacturing of metal products.
