Investigation Summary
At approximately 7:30 a.m. on March 25, 2000, Employee #1 was placing a piece of photo plaque into an in-running roller press. Employee #1's right hand was caught under the plaque and pulled into the nip point of the in-running roller crushing her middle, ring and small finger. Employee #1 suffered an amputation of the right ring finger distal phalanx and whole small finger. Employee #1 was hospitalized.
