Investigation Summary
At approximately 10:50 a.m. on April 9, 1999, two employees were working in a metal-recycling yard. Employee #1 was standing near a truck-trailer platform while a coworker was operating a forklift. The coworker was backing up to reposition the forklift in order to dump a load. The forklift moved toward the platform on a perpendicular angle and pinched Employee #1 between the rear of the forklift and the truck-trailer. Employee #1 received crushing injuries to his arms and chest and died that evening.
