Employee's finger amputated in rollers of banding machine

Employee #1 had worked at a metal stamping facility for three years. He was working at step three of a four-step band production line when he noticed that a coworker, who was working at step 1, was having difficulty with his band-forming machine, model 159, serial #1989. When Employee #1 went over and stood in front of the machine's rollers, he noticed a piece of sheet metal lodged between them, causing the line to jam. The machine had 10 rollers, spanning approximately 8 ft in length, and located 43 in. above the floor. The lodged piece of metal was in the eighth roller. Employee #1 set the rollers to what he believed was reverse mode, and activated them. His glove became caught in the rollers, which were actually moving in a forward direction. His right hand was pulled in and he suffered a partial amputation of his index finger, to the distal phalanx. Employee #1 had not been trained in how to unjam the machine.
