Employee's skull fractured when struck by flying hook

At approximately 2:30 p.m. on August 4, 1997, Employee #1, of The Union Ice Company, Los Angeles, CA, was removing an old ice-drying belt. He fastened the metal hook with nylon rope and a link of the chain. When the rope was pulled backward by a forklift truck, the link broke and the flying hook struck Employee #1 on the head. He suffered a fractured skull and was hospitalized for 12 days and then treated as an outpatient.
