Investigation Summary
At approximately 6:00 a.m. on April 10, 1997 Employee #1, of the International Baking Corporation, was greasing the chain and sprocket drive of a pretzel machine. He removed the machine's guard, and was greasing with the chain and sprocket in motion when the tip of his right index finger was caught in the drive and amputated. Employee #1 was hospitalized at Stacy Medical Center, and then treated as an outpatient.
