Investigation Summary
At approximately 3:45 a.m. on October 2, 2006, Employee #1 was servicing a snack cake mixer at Interstate Brands Corporation, a manufacturer of snack cakes. He was clearing a transfer pump that was jammed and had not locked out the unit. Employee #1 was injured when the machine operator started the unit while Employee#1's middle and ring fingers were in the machine, sweeping material away from the opening of the transfer pump. Employee # 1 was hospitalized for the amputation of his left middle and ring fingers.
