Investigation Summary
At 12:30 p.m. on August 15, 2007, a truck driver was loading pallets of produce on his truck using an electric pallet jack. As he loaded one of the last ones on, he went to step off the pallet jack and his foot slipped off the jack and became wedged between the wall of the truck and the jack. Simultaneously he turned and activated the control of the jack, moving it slightly, snapping his ankle, in a compound fracture. He was hospitalized.
