Employee amputates finger in machine blade

At approximately 6:20 a.m. on October 23, 2003, Employee #1, a factory worker, was working for Friedman Bag Company in Los Angeles, California. The employee was cleaning and adjusting the Automatex Bag Maker Machine Number 3 because the glue used in the bagging process was creating problems. The machine was stopped and the employee had his right hand under the side guard opening to catch the bag material. He used his left hand to use the Jog Forward control located on the panel cutter console. On the panel cutter console there are several control buttons, for instance, Jog Reverse, Jog Forward, Shear Jog, Start, Stop, Emergency Stop, Scanning Adjust, Jog Speed and Unwinder Speed. The Jog Reverse and Forward buttons are located next to the Shear Jog button. The employee was trying to push the material forward by using the forward control button; however, he pressed the green Shear Jog Button located next to the forward control, button which cycled the blade. The blade rotated and made contact with the employee's right hand, which was under the guard opening at the point of operation. His right index finger was amputated. Employee #1 was transported to White Memorial Hospital where he was treated and released but not hospitalized. According to the accident report, the Automatex Bag Maker was not stopped and the power source de-energized or disengaged. In addition, the Panel Cutter Blade was not mechanically blocked and locked out or otherwise secured to prevent inadvertent movement during cleaning, servicing or adjusting operations.
